The Transition Care Program provides time limited, goal orientated and therapy focused intervention to older people who require more time to complete their restorative process and optimise their functional capacity following hospitalisation. The Program aims to support older people with the potential to benefit from TCP with individually tailored therapy, nursing care, and activity of daily living assistance to enable older people to further improve or maintain their physical, cognitive and psycho-social functioning. This program helps older people get back on their feet after a hospital stay. It provides short-term care for up to 12 weeks, including social work, nursing support, personal care and allied health care.
